Edith Jones-Poland, MD

Bio
Dr. Jones-Poland believes that hearing about her patients’ lives is one of the most rewarding parts of being a Primary Care Physician. She enjoys working in tandem with each patient to help them become as healthy as they can be.

Her medical practice philosophy is to treat the whole person: “I enjoy science and have a heart for service. Medicine is the perfect marriage of the two. Healthy people build healthy communities.” Dr. Jones-Poland focuses on disease prevention, not just treatment, through the most natural and least invasive means possible. Following medical school, she completed a Fellowship at the internationally recognized Arizona Center for Integrative Medicine, founded by Dr. Andrew Weil.

Dr. Jones-Poland is an ardent supporter of Go Red for Women, an American Heart Association initiative created in 2004 to end heart disease and stroke in women. In 2021, she was chairperson for the program in the Coachella Valley and Hi-Desert areas.

Believing that “food is medicine,” Dr. Jones-Poland’s passion for fitness and nutrition is evident in her practice and life. Not surprisingly, she likes to cook, exercise, read, and spend time with her husband of 16 years – a professional musician – and their four daughters.

Gabrielle C. Schwilk, DNP, FNP-BC

Bio
Gabrielle has accumulated more than 10 years of experience as a nurse practitioner and has served in the healthcare industry for about 25 years. She believes knowledge is the key to better health. Gabrielle shares her knowledge by educating patients about the need to complement traditional medicine with homeopathic care.

“Understanding the complexity of your health can be difficult at times, especially when it comes to health conditions and how to manage them. It’s important for me to take the time to explain it in a way that’s easy to understand so my patients can take part in maintaining or improving their own health.”

Gabrielle has two adult children and enjoys caring for patients of all ages.

Raeanne Holder

Bio

Longtime Yucca Valley resident Raeanne “Rae” Holder is a nurse practitioner with nearly 15 years experience in healthcare. Before graduating with top honors from Chamberlain University as a family nurse practitioner, she worked as a registered nurse in the specialty areas of cardiology and oncology.

Rae listens to the concerns of her patients and is committed to staying up to date on current healthcare practices to keep you healthy and thriving. She is driven to provide competent and compassionate care that patients deserve.

When not with her patients, Rae is a mom of 3 boys and the wife of a USMC Veteran. She loves to sing at her local church, swim, and coach her boys in basketball.
